Multiple causes are identified and being worked :

Case 1. Multiple media with the same OST media label exist in deduplication storage. The cause is being investigated.
Case 2. If any media from the source set is corrupted, it would lead to consolidation Full failure with the error mentioned.
 

Workaround :

Case 1 - It is suggested to open a case with Backup Exec Technical Support.

Case 2 - A full backup stage can be added to the existing backup job. Run the full backup stage to create a new full backup set. Make sure the full set and incremental set are error free. 

Note: Backup Exec server should be rebooted when no backup jobs are running.

This issue is presently under investigation by our Engineering Team. Depending on the results of this investigation, it may be addressed through a patch or hotfix in current or future software releases. However, this specific issue is not currently scheduled for inclusion in any upcoming release.
