Cause

This issue occurs when upgrading from System Recovery 2013 or 2013 R on the same install path.

Note: To successfully upgrade from System Recovery 2013 or 2013 R2 to System Recovery 16, System Recovery 16 needs to be installed on the different install path from that for System Recovery 2013 or 2013 R2.

Resolution

WORKAROUND:
If the issue occurs, please repair using System Recovery 16 install media.

After repairing, System Recovery 16 console can be opened successfully.

  1. Insert System Recovery 16 install media.
  2. Run BROWSER.exe.
  3. Select a language and press "OK".
  4. Click "Install Now".
  5. Select "Repair" and press "Next".
  6. Press "Install".
  7. Reboot your system after the install wizard completes.
